How to display Employee Assignment details on DOR request page?

The details are needed to show on DOR request page and notification as well like:

*Employee Number,

*Employee Name,

*Department,

*Position,

*Job &

*IQAMA Number.

The attachment I added, where I need to see the employee assignments which were mentioned in the description.

The details are to be shown both on DOR request page and Notification page as well.
