Redwood Learning - Error when creating a training supplier/instructor

Has anyone else had an error when creating a training supplier or instructor (if you are using Redwood Learning) and you try to add an attachment/URL when creating that supplier/instructor?

Steps:

My Client Groups>Learning>Catalog Resources>Training Suppliers

Add new training supplier

Select an attachment / add an URL

GLITCH: the attachment/URL is vaguely visible and flickers at the bottom of the Related materials section.

ERROR: Click Add and the error appears, supplier cannot be added.

Cancel out of the screen.

Workaround:

Add the new supplier again and this time do not add attachment/URL - you can successfully add the supplier.
