Within OTM cloud, how do they really update a row in a table?

Summary: In OTM cloud, when you try to 'update' a row in a table, are you actually doing a 'delete and then insert' instead? Are you doing that to ALL tables? or just SOME tables? All the time? Or just in certain conditions? Could you please clarify? Could you please give the list of the tables you are doing that? We suspect you are actually doing a 'delete and then insert'

Background info why I am asking this --- our data ware house team noticed, and due to this, they are missing some critical info on their end. We are looking for options we have that could give us this piece of critical information.
