While Upload Rates the Upload Rate Record is null in Approval Summary

Content (please ensure you mask any confidential information):

When I upload rates, the rate record is not displayed in the Approval Summary and Details. Status is "PARTIALLY SUCCESSFUL".

When I download the failed rates I´ve the error message "java.lang.NullPointerException" in Rate Record Structure.

Does anyone know what could be causing this?
