Getting error on flex form Index:3 Size:3

Summary: When we try to save data on a flex form from smart view, it throws the below error

Content (please ensure you mask any confidential information): There is a groovy rule which runs a smart push attached to the form, the rule gets triggered and runs successfully but we get this error and the state remains as editable (yellow) in smart view. When we refresh the form, we get the same error but when we reopen the form the updated value is visible. Has anyone faced this issue and can guide how to resolve?
