Learning Assessments are not marked as completed

Hello,

Some of our users are experiencing learning assessments that are not marked as completed, even though they are getting a passing score. We tried the reconciliation but it didn't work. Some of them are able to complete the assessment either after clearing browser cache and cookies or take the assessment twice or thrice with the same selection of each question.

Have any of you ever had this problem?
