bulk item upload through input interface not working

Summary:

bulk item upload through input interface not working, we are trying to upload bulk item using ITM template through manual UI -input interface , it always upload only 1 item which is populated in last sequence .

I am using attached format , pls guide if there is issue in Template .

As per template you can see i have added 2 item , but only 2nd item got uploaded , not the other one
