OIC Notification action not sending email, but showing success

Hi all,
I am trying to send email notifications to users within an OIC integration. it's a simple "this integration is successful" email to multiple recipients and sometimes emails are sent, but sometimes emails do not send. How do I prevent this from happening? I don't mess with the integration and it will send emails for a couple of days and then just stop sending out of no where even though when I check the logs, it says an email was sent.
I am using "no-reply@oracle.com" as the "from".
