Ability for employee to self report a conflict of interest.

Summary:

I have a requirement for an employee to be able to self report conflict of interests, including employment of relatives, material interest and outside interest.

The employee initiates the conflict of interest and provides details including start date, brief description, etc. and a workflow would be required.

Currently, we are exploring options of creating document types for each of the conflicts that would open fields to be populated by the employee and initiate a workflow, however, we are interested if anyone has a more delivered functionality solution.
