Can you add Dynamic fields to the Chat launch page

We would like to be able to add three fields dynamically on the Chat launch page when the end user selects a particular issue from our drop down.  Thus making the end user to provide required information that is only needed for this one issue.  Is this something that can be done and has anyone done it.
