Getting REST ACTION ERROR when attempting to add document records (Manager)

Summary: Manager attempting to add Document Records for themselves and their direct reports and receiving error "Error Response during RestAction" - does not matter the document type, it is happening for all document types when attempting to add for themselves and their direct reports. Page was recently updated to be Redwood.
