Why is it that while approving ERES record in oracle quality, occasionally ERES record shows as none

Automatic Generation Failure: Sometimes, when a user performs an inspection, the ERES document does not generate automatically as expected.
Manual Generation Issues: When the document is manually generated using the Schedule process, it is created but lacks the required approval signatures.
