How we can approve or reject the approval tasks in new Approvals Application?

We really like New Approvals Application and want to use it. We have followed all the steps in the documentation. However, while we can see the approvals, the pages do not refresh when we approve. This issue occurs on both web and mobile. We are unable to approve using the Approvals application. When we try to approve or reject in the application, we get the following error message:
