How to find Grade ID for Employees

Hi,

I am trying to complete a Bulk Load and require 'Grade ID' for employees, I have the Grade Name and Grade Code but not the Grade ID associated, how can i extract this using a MI analysis or SQL? If anyone has a query they have used for this that would be great :)
