Self-Paced Learning - Questions and Answers greyed out and Learner unable to answer

Having an issue where colleagues are completing a mandatory learning and when it comes to answering the questions it is giving them answers to a previous years course.

We are having at the moment an issue that seems to be happening randomly to colleagues on our Mandatory self-paced learnings.

They are working through the course and when it get to any part where they are to answer the questions they are unable to as the questions are grey out with what seems a previous years answers.

It is a completely new scorn files we have received and have built a new course with the new scorn files attached so are unsure how it is pulling this data and why its happening randomly.
