Annual training assignments duplicated for learners

We are seeing duplicate training assignments for learners when they are taking their second annual training assignment. Last year they were assigned with a retraining cycle. This year they get the assignment twice. If they finish the assignment both times, one assignment is then generated for next year. We wanted them to have the training due every July 31 and have the assignment open 30 days ahead of that.

What did we do wrong here?
