Learning completion shows data from prior completion rather than latest completion

I have courses that have a yearly retraining assignment. When the people are going back in to take the training, some of them show completion information from last year. For example, last year Learner X took the training on July 13, 2023 and got an 85 for a score. This year they took the training on July 19, 2024 and got a 96. The system recorded the same date and score for both completions and then generated the assignment for 2025.

I'm also seeing some users where they have the same score recorded twice, but the newly generated assignment is again for 2024. If they take the training a second time for 2024, then they get the 2025 assignment generated. However, not all learners are seeing the same thing. Some are working just fine. And yes this is happening on multiple different courses.
