Duplicate actions are showing in global search

Summary:

As an employee I want to be able to easily jump into a self-serve action using global search returned results that are simple and easy to follow.

Currently at First West we are seeing duplicate results (more in our DEV pod than production) which is making the list longer and more intimidating than it needs to be. Is there a job that needs to be run to refresh indexes or something here? How do I remove the duplicates?
