How to add survey banner when user login

Hi Folks,

I need to add a survey banner when a user login as a popup. Please help me hot to add this.
While login i need to have this popup and after submit it should take to the login page. Please help me.

Below is the reference screeshot of the survey. The circle with yellow color back of the popup is the home page. Once he submit he then homepage should open.
