Create pop up screen when users log in as survey.

Hello All,

is there's way we can create pop up survey, when employees log in to fusion.

and user must click on it submit and then he can enter to homepage.
Below is the reference screenshot. And the one circled with yellow is the home page back of the pop up. Please let me know how to achieve this.
