Termination workflow not sending notifications

Summary:

Termination workflow not sending notifications in lower instances.

We have a requirement to send email notification to IT whenever an employee is terminated.
Though the notifications were turned on and Information-Only Approval Groups were added, no notification emails were triggered.

The transaction console has a record of this event as completed but I do not see this displayed in 'Notifications Created by Me' or in the Worklist app.

Upon testing other flows, we noticed that notifications are being triggered for those waiting on approval and not for those having a self-approval.
