Which table stores the acknowledgement date of performance document and comments entered by employee

Summary:

We are trying to customize one performance module seeded report. In the report, we are trying to fetch whether document is acknowledged by employee or not. If yes, then we need to get acknowledgement date and comments entered by employee while acknowledging the document. We have queried multiple tables but couldn’t find the table where all this information is getting stored.
