Assessment is showing Completed in the Past Even with Must Retake Assignment

Summary:

We do annual training and randomly people who get reassigned in an Offering with the "Must Retake this Assignment" will have the Assessment already showing Completed from their previous attempt.

I am not sure what the point is of "Must Retake" if it is going to automatically give them completion on the older assignment.

We also have an issue as you can see below that the "Launch" button disappears which I have recently opened SR for.

Anyone have any ideas why either of these happen? What is best practice in setting up annual training that must be retaken?
