How to retrieve audit reporting details concerning attachments like f.ex. Document of Records

Hi,

Our client would like to have an Audit Report that generates the creation, update or deletion info of a Document of Record (with related user data of course; resp. who created, updated or deleted the record).
Preferably they would be able to retrieve the details of the DOR (or other attachment) specifically as well.
Is this possible? If yes; Could you please specify how or provide us with detailed instructions?
