Receiving multiple notifications for one transaction

An add pending worker that gets auto-approved by the workflow system is shown many times for the initiator.

I have tried changing the Task Aggregation in BPM Worklist to Once per task and Once per stage but still receiving the multiple notifications. Is there any way to avoid this?
