Issue with Emergency Contact Flag in Family and Emergency Contacts Page

Hi Team,

On the Family and Emergency Contacts page, we customized the Emergency Contact flag to be checked by default(Attached the screenshots of customization). In the UI, we can see that the value is checked by default. However, when we create a contact and clicked on submit, the Emergency Contact field displays as "No." Could someone please help us understand why the field is checked in the UI but shows as "No" after submission.
