Reminder emails are resending to approvers months/ year after action has already been completed

Has anyone experienced emails being resent many months after the action has been completed?

In one case, an award/ project contract was amended to terminate on 7/2023 and all approvals have been completed. On 9/2024 a reminder Action Required for Approval was resent to the approver.
