How to properly set up advanced controls for requesting/provisioning roles

We are trying to use the Risk Management feature of advanced controls for requesting/approving roles. We have assigned the requestor and approver roles and enabled the profile option, however when we submit a role request it just sits in "Awaiting user validation status" and we can't find where to validate it so the role gets added. Any suggestions on what we could be missing?
