Approval rule does not trigger FYI notification as the first step

Summary:

I have setup the resignation approval rule as shown below -

In this example, both "Application Role" steps are "FYI Notification" and "Management Hierarchy" step is "Approval Required".

So, when testing, FYI notification is NOT triggered as soon as the resignation is submitted, however the 2nd FYI notification is correctly triggered after the manager's approval. Can you help why?
