Unable to configure Lease approval

Summary:

I was able to configure Lease approval but not working as expected. I want to use Approval group and Supervisory but whenever user create lease it is showing as submitted for approval and it just go to one of the user mentioned in approval group and not user manager or to another user in the approval group. Not sure what can be wrong.

I tried to create with Task Creator but system is not accepting and throwing an error.

Also i was not able to view under Transaction console and let me know if any tables i can query to find the details of workflow.
