Users Unable to Login without Resetting their Account

Summary:

Is anyone else having issues with users being unable to PROD log in until their user account is deleted and re-added? We have had a few come up in the last month or so which is an increase for us. In one instance the user started experiencing issues after a name change. The other cases have been our international team members trying to get into the system. When they go to log in, they get the dialogue box in the screenshot below. Any insight is welcomed!
