Program Documentation Report isn't showing source, destination, and driver basis

Summary:

When I run the Program Documentation Report, I used to see the Source, Destination, and Driver Basis columns populated for each rule in the report. Now when I run it, those columns are blank for every rule.

When the report is run, the beginning of the report looks as expected:

However, when I click one of the rules, I receive a column with nothing in the Source, Destination, or Driver Basis columns

Reports that were run previously contained this data (see below for a populated example for the same rule for the same POV).
