Agent failures with Unique constraint errors

Summary:

Hi Team,

We are seeing Unique constraint errors in the Agent workflows in majority of our agents.

Although we are ensuring that the existing refnum is deleted and then reinserting the refnum, but such errors still pop up.

Is there any best practices to avoid such errors apart from Deleting and Inserting again? Let me know.
