Ad Hoc Grid Action Menu - Forms not opening?

Summary:

With the new "Enable Action Menu for Ad Hoc Grid" feature introduced in the December update, I am struggling to get this to work for forms. When I set up an Action Menu associated with a specific form and attempt to use the menu item, it gives the below error. I can open the form as normal without using the Action Menu item though. It is as though it is not applying the application connection correctly when trying to open the form. Has anyone had success opening forms from an Ad hoc Action Menu? Thanks for any help!
