Exception 'An item with criteria group ids **** already exists'

Has anyone seen an exception like this :

[119340135] An item with criteria group ids=[DOCUMENT: MOWI.******-5607256-0-LATEST, Integration: 0009455466], sequence=T0173009898 already exists in group DOCUMENT: MOWI..******-5607256-0-LATEST

The document is created, we can view the binary, transmission is processed, but it keeps throwing this error. It`s not really a problem (since document is created), but it worries me if that could become a problem. Something is off somewhere and would like to fix it somehow so we dont get exceptions.

Document Transmissions are staged before they are processed. Recurring process is picking the staged transmissions and when it`s ok it does one thing: 'Process transmission'.
