Initiative stuck in submitted state with config of Auto accept responses

Summary:

Hi All,
We have an initiative with type 'Assessment' created. For all the qualification areas included in the model, we have set control of 'Auto Accept responses'. At the initiative questionnaire controls also we have selected the checkbox for 'Automatically accept responses'. But after the internal responder submitted the responses, the initiative is stuck in 'Submitted' state and not moving ahead to 'Accepted' state automatically.
When we tried to create new initiatives with the qualification model, it goes through. We are trying to understand why that particular initiative got stuck. Are there any log files/processes that we can analyze? Why did this issue occur isolated for a single initiative?
