How to Get Response Text for Performance Document Questionnaire

Summary:

Have a report I pulled from Shared Reports as it gets me closer to what I am trying to pull, but it still leaves a blank in the Response Text section.

I am trying to pull anyone with certain answers to our "Step" question on a Performance Document Questionnaire. I can filter the "Question Text" to what I want (Step), but the Response Text is blank and I can't filter to the different items that are in that dropdown.
