Capture list of all Resources & their Roles with corresponding Provisioning Method?

We need a way to identify all Active Users in CX Cloud that will display ALL Roles that have been assigned to the Resource AND which Provisioning Method was used…automatic or manual.

Does anyone know of a way to retrieve this information from CX Cloud? I checked OTBI & can get a report of Resources & their Primary Resource Role but not one that shows ALL Roles. I also do not see any field that would denote the Provisioning Method in OTBI.
