Report for Confirm Receipts process

Is there a report that can be built based on the responses received for notifications on the Confirm Receipts process?

The report required is to identify the PO Notifications on which the Requesters selected the "Did not Receive" Option and the PO Notifications whichever got escalated to the line managers.

Which backend table stores these data so that we can build a report?
