How do I add custom fields at layout CHANGE_SERV_PROV?

Hi, users need to include custom fields in the screen when they run "Change Service Provider" actions (From shipment Management), By default OTM displays the list of available Carrier, and cost. But we also no display attributes fields and calculate them.
