Termination request being approved before last approver action

Hey!

We are facing an issue where the workflow approves before the last approver takes an action. In this scenario we have the same responsibility as approver for 2 consecutive steps:

The workflow is set like Requestor > Informative to manager > Informative to BP > Termination Team AOR > Termination Team AOR again.

Task aggregation is already set to none in BPM and there is no autoapprove set for the specific rule triggered for the request.

The person that sent the request is part of the Termination Team responsibility, so the requestor is also receiving the approval notification.

I never saw anything like this before, if anyone know what may be causing this please let me know.
