Mass Retirement of assets configured, approval status indicated "required" but not initiated.

after adding the following details in the workflow sheet, the status for mass retirement "approval status" has changed from Not Required to Required. but no approval is initiated. (the other unfilled columns are hidden) please advice if any other column needs to be filled and what the possible cause of the approval failing can be?
