Report on Deleted Interview Feedback - Recruiting

We have a requirement to track the Delete Feedback action on Candidate Job Applications. Has anyone created a report to do this?

We would like to create a report to run an audit on a regular basis to see:

Candidate ID
Req #
Questionnaire Name
Action = Delete
Actioned By Name
Actioned on (Date)

I have tried creating an OTBI report but I can't find columns related to Action 'Delete Feedback' and User details / date etc. I'm thinking this is likely a BI Report so I'm looking for feedback from someone who has created a report for this requirement or something similar.
