Change order PO stuck in approvals

Summary:

we have issue with Change order approvals in procurement. After reviewing the code below in BPM Task, we have identified that the rule is meeting all the logical requirements but when it hits the action part it fails to generate the approval hierarchy for the change order at "Starting Participant: HierarchyBuilder.getManager("supervisory",PurchasingDocumentDistribution.requesterOnPoUserName,-1,"","")". I was just wondering if anyone else has come across this issue? Any suggestions to fix this issue will be highly appreciated.
