Hide fields on the Benefits self service Contact screen

Summary:

Hide the Disability Status, Tobacco Use and Student Status fields on the Contact screen. This can be done in both Benefits Self Service (BSS) and Benefits Service Center (BSC).

We did this personalization, but it is not working. We can still see Disability status and other fields appearing on redwood UI when we are creating new contact.
