Does anyone have an excel template example I can use for bulk assigning user roles?

Summary: I am trying to find a simple excel example of the User.Dat file that I can use to apply user roles to users on mass. The criteria for applying these roles does not fit with using the auto-provisioning option so I would like to do this using the data loader but I cannot find a simple example of the column headings I need to use.
