Getting error when using Digital assistant

We have configured Digital Assistant and the embedded tasks were working. However, I am now getting an error message for any command I ask.

Content (please ensure you mask any confidential information):

"Unexpected error occurred, please contact the helpdesk" is the response regardless of the command. Previously, the commands worked - what could have changed?
