OIC instances stuck "In progress"

I am reaching out to see if anyone else has encountered an issue we've been dealing with since March. Although the problem was initially severe, patches and OIC updates have significantly reduced its occurrence. However, it still happens occasionally. We have an open service request for this, so I'm not necessarily looking for a solution, just some feedback.
There are periodically integration instances that do not complete. If I go into "Observability > Instances", sort by "Creation time", and filter by "Last 32 days" and status "In progress", I see instances that started days or weeks ago. Right now I've got two instances that started on Sep. 12th and 17th. Often they have made it to the very end of the flow, but we've also seen them get "stuck" on steps within the flow. We have not been able to find any pattern or commonality (trigger type, synchronous/asynchronous, time of day, etc.). They do not end in success
