in Redwood work definitions are duplicating on the list

We noticed today that one item was listed 7 times on the Work Definition listing using Redwood. All the information is the same for each one. There is only 1 Work Definition created.

We noticed today that one item was listed 7 times on the Work Definition listing using Redwood. All the information is the same for each one. There is only 1 Work Definition created. On the old UI, the Work Definition also shows once. Any ideas what could be causing this? It is very confusing to the users.
