Why is document of record there (three dots) but cant view?

Summary:

I need HR/Manager to add a document to DOR and then employee to view it. Ive tested but when I add the document its behaving very strangely, there are 3 dots indicating its there but I cant view it, this is as Employee and also even someone with HR specialist/ Security Manager/ Line Manager etc
